Do I need to have the separate the separate voltage sensor I bought separately plugged into FrSky D8R or not?

Only if you are using a high voltage pack.
A1 should be showing a value of the built in voltage divider.
Do you see rssi values?
Have you pressed long down button to the telemetry display screen? (Is that how it still worlks with er9x?)

In setup page 11 the maximum I can get using the A1 Voltage multiplier is 25.5 which gives me a voltage to the right of 9.6
The actual voltage of the battery is 11.1

What is connected to the A1 plug of the receiver? An external sensor, a jumper cap or nothing?

On the bottom of the FrSky D8R is pin 1 jumpered

Then you are measuring the receiver's power supply. This will usually be a constant 5V, as supplied by your ESC's BEC.
If you want to measure the pack voltage you need an external sensor.

the receivers (remote control) power supply is 12 x AA batteries over 10 volts

Can you make a photo of your setup?
As said unless the 12AA are connected directly to the receiver (which would likely burn your servos), you likely have a regulator in the path.

I have a spare FrSky D8R and put that in the plane
Now on my Telemetry screen I am showing
9.6v
under this is 9.6v & 9.6v
I have been running the motor for 5 minutes but no change in the voltage displayed

the TGY 9x is powered by 12 AA batteries

The plane is a bixler with a Turnigy 3 cell battery that is 11.1 v which I am tryig to read

You will need to use the external sensor, plugged into the A2 port.
The A1 is reading the voltage to the receiver from the esc. This is normally around 5v.

12 AA batteries powering the tx? How did you fit them in? More like 8 I reckon, giving you the 9.6v.

As Kilrah suggested, please post a couple pictures from your setup..
Don't connect your flight battery directly to your receiver.. you will burn it and your servos if you do it..
As said before you might be powering your receiver from the ESC (motor controller) and this would be 5V. due to the shunt on the A1 input on the side of the receiver you will see this voltage on the screen. In this configuration the receiver is measuring it's internal voltage.
Go to the telemetry configuration menu, scroll down to A1 and change the range to 13.2, and you will see then the correct receiver voltage on the telemetry screen.
To access this screen in er9x, press the DOWN button several times until it shows the telemetry screen. You will also see the RSSI values which are the RF signal strength.
For displaying the plane's battery you will need to connect a voltage sensor between your battery's balance charge connector and the A2 input connector on the side of the receiver.

It's been a while since I've added a frsky voltage divider to my system. i have a new plane and thought I'd ask.
Are we still NOT connecting the - lead from our plane lipo pack into the divider to stop and ground loop errors?
I'm using the deans male/female inline connector with the JST tap on it. I add the mating plug onto the pigtail ends of the Frsky divider leads. In the past I just cut the - lead/black where it enters the Frsky divider a short way up from the pc board so I can reconnect it if i have to. As usuall, I uses both + and - leads into the RX data port.

For me: + and - of the battery to divider, and only the middle of the divider to the RX (no -). That way the current path through the divider doesn't have to go all the way through the receiver and the ESC.

Installed Voltage sensor OK , a bit fiddly to get it connected to the battery
